Community Partnerships is a nonprofit organization serving children with mental health issues to be successful in their homes, schools, and communities. Community Partnerships currently provides the following programs and services: Children Come First—An intensive case management program serving children in Dane County with mental health issues who are at risk of institutional placement. Case Management Services—Intensive case management services and wraparound support for children living outside of Dane County. The Engagement Project—A collaboration with the Madison School District and Memorial High School, working to successfully engage incoming freshman with an overall goal of increasing graduation rates for at risk youth. ArtSpeak—A therapeutic expressive arts group for children and families. Family Advocacy & Support—Parent and family support, including monthly Parent Meeting groups.

The Wisconsin Primary Health Care Association (WPHCA) is the member association for the 19 Community Health Centers, 17 Federally Qualified Health Centers (FQHCs) and 2 Look Alike Health Centers, across the state. WPHCA provides T/TA to Health Centers for Quality Improvement, Health Information Technology, Special Populations, Chronic Disease Management Systems, Health Policy, Advocacy, and more. We are proud of the care that our Community Health Centers provide to nearly 300,000 Wisconsinites each year.

Pathways of Wisconsin Inc, is a non-profit vocational agency dedicated to providing, person centered, quality services to adults living with developmental disabilities in the Madison area. In 1985 Pathway's goal was to help integrate people living with disabilities into the general workforce. We work closely with the Division of Vocational Rehabilitation to maximize each individual’s career goals. Pathways also offers a small day service program and community activities for those people who are in between jobs and/or do not have employment as their goal. These activities include, but are not limited to: art enrichment, movement therapy & exercise, sensory activities, and skill building. We maintain a small day service space that people may use when they are in between activities/jobs and or need a familiar safe space. It is the mission of Hope House to end homelessness and create healthy communities. Hope House is an emergency and transitional living facility as well as a true community center located in the near south side of Milwaukee. Opened in 1987, Hope House was created to provide a safe, temporary refuge for people needing a place to sleep. Over the years, Hope House has evolved into a complete provider of services for homeless and low income individuals. In addition to residential services, Hope House provides community outreach to individuals needing health care, food distribution, financial assistance, and educational opportunities for adults and children. Hope House provides services that aim to end homelessness and create healthy communities.

The Cathedral Center, Inc. is a nonsectarian agency for women and families. The mission of Cathedral Center is to provide a safe environment for women and families while working to end homelessness, one life at a time. Cathedral Center can provide emergency shelter and comprehensive case management services for 32 individual women and up to 8 families on any given night.

Wisconsin Right to Life works to build a culture that values all human life through education, mass media, legislation and the political process. (WRTL airs life-affirming digital ads under the name Veritas Society.) The organization has not only worked to pass every life-saving law currently in the Wisconsin statutes, but has authored most of them. In fact, WRTL is so experienced at writing right-to-life legislation that not one of its laws has been overturned by a court of law. WRTL leads the nation in providing groundbreaking, highly successful programs like our grants to college students, teen leadership training camps and mass media television campaigns. These programs are unique to Wisconsin and along with sweeping legislation are saving lives. Wisconsin's abortion ratio, the number of abortions to 100 live births stands at 12 in 2009, half the national average of 24. It is the oldest and most successful right-to-life organization in the state and one of the most successful in the nation.

Serving Older Adults of Southeast Wisconsin, Inc., also known as SOA, is a new Wisconsin non-profit organization founded in 2018 by a team of experienced and passionate professionals dedicated to serving the needs of older adults. With decades of collective experience in providing older adult programming, SOA founders developed the organization as a means to provide proactive and positive approaches to aging. In doing so, we strive to help older adults maintain optimal health and independence for as long as possible. Our hope is that older adults will spend more of their years supporting their needs socially, financially and physically while adding value to their families and communities. We also run five senior centers in the Milwaukee area funded by MCDA.
