An idea to change healthcare in the Mahoning Valley… The Surgical Hospital at Southwoods was created in 1996 with a singular purpose – to create a healthcare facility for our community that placed importance not just on providing excellent medical treatments, but also how patients were treated. Initially focusing on outpatient services, Southwoods established a new standard of healthcare. The Primary Health Network began as one small community health center site in the Shenango Valley in 1984, with a central focus on providing the highest quality medical care to the people we serve. Since our inception, we have worked closely with our local communities to implement a new model of healthcare: the Community Health Center. After 37 years of service, PHN has grown to become the largest FQHC in Pennsylvania and one of the largest in the nation. Currently, we staff over 150 physicians, dentists, psychiatrists, physician assistants, and certified nurse practitioners. Alongside these professionals, PHN is staffed by over 450 support members. At PHN, our patients are the center of our focus. We have taken various measures to ensure that our patients are receiving the highest quality of care. Some initiatives include our Primary Care Medical Home and our continuing accreditation by the Joint Commission. In 2020, we reached over 70,000 patients through our health and educational services. These patients can receive care from one of our 50 sites in 17 counties across Pennsylvania and Ohio. We offer comprehensive primary care and numerous specialty services, as well as working within communities by participating in health fairs and networking with local organizations. Our management is structured to engage and interact with communities, as the Board of Directors is composed of volunteer community leaders and residents throughout PHN's service area. Founded in 1993, Signature Health is a Federally Qualified Health Center (FQHC) providing mental health, addiction recovery, primary care, infectious disease, pharmacy and lab services to patients of all ages. With 10 outpatient and residential locations in three Northeast Ohio counties (Ashtabula, Cuyahoga and Lake), Signature Health primarily serves Medicaid and Medicare patients and offers a sliding fee scale based on household income for patients without insurance. Committed to putting good health within reach of all our community members, Signature Health increases access to care through telehealth, walk-in hours, evening appointments, transportation support, on-site laboratories, and on-site pharmacies with free mailing and delivery options.

Our art and music therapists serve the psycho-social needs of our patients with end-stage renal disease as they adjust to and cope with life on dialysis.

The Kidney Foundation of Ohio was formed in 1950 by a group of parents whose children suffered with childhood nephrosis. Incorporated and determined by the Internal Revenue Service to be a 501(c)(3), Not-for-Profit Organization in 1961, the Foundation celebrates 67 years of service to the community in 2017. The Kidney Foundation has gone through several transitions over the years, including affiliation and later disaffiliation with the National Kidney Foundation. As time went on, the Foundation grew in size, eventually covering 32 counties in northern and eastern Ohio and incorporating three chapters. Currently, we provide assistance to not only 32 counties in Ohio, but five counties in northern West Virginia, as well as chapter affiliates in Lake, Summit and Wayne Counties. What was once a small group of families has become the premier agency in Ohio for providing direct assistance and education to those suffering the effects of kidney failure. The Kidney Foundation remains one of the few health and human service agencies in northeast Ohio which continues to provide direct dollars to patients to assist them through the devastation of kidney disease. The Foundation remains an Ohio-based organization and all funds raised in Ohio, stay in Ohio.

The fact is that organ, eye and tissue donors and recipients are more than statistics or medical stories. They are our friends, our neighbors, our families – they are our community. Lifebanc serves 20 counties in northeast Ohio and works with 80 hospitals and two transplant centers. Lifebanc is a member of the Organ Procurement and Transplantation Network (OPTN), which has been managed by the United Network for Organ Sharing (UNOS) since 1986. Lifebanc is proud to be a local host of the upcoming Donate Life Transplant Games - Cleveland June 10 -15, 2016.
