Third Street Family Health Services was founded in 1994. We are a network of non-profit community health centers in northcentral Ohio. We improve the health of communities by helping to remove barriers to wellness. We provide behavioral health, community health worker, dental, medical, medication-assisted treatment, OB/GYN, pediatric, pharmacy, and podiatry services. The Joint Commission is the nation’s oldest and largest standard-setting and accrediting body for health care. All of our departments are committed to sound management practices in the day-to-day delivery of safe and high-quality patient care.

Grace Hospital is a registered Acute Long-Term Care Hospital, the first of its kind in Cuyahoga County. We are Medicare & Medicaid certified and fully accredited by the Joint Commission. As an Acute Long-Term Care Hospital, our reimbursement from Medicare & Medicaid is based on Acute Care Days, rather than Skilled Nursing Days. Our services are also honored by most private insurers. At Grace Hospital we offer 24-hour monitoring of every patient, provided by a fully certified nursing staff. Our nurse to patient ratios are dependent on the conditions of the patient. Physicians and specialists are always on call, so that any situation or crisis can be resolved quickly and skillfully.
