Celebrating life since 1979, Transitions LifeCare was founded as Hospice of Wake County in 1979 and provides physical, emotional and spiritual care to those living with an advanced illness, their caregivers and those who have lost a loved one. Our services include hospice care, palliative care, pediatric hospice and palliative care, caregiver support, and grief counseling. Services are provided regardless of age, race, religion or financial circumstances. Our service area includes eastern Chatham, Durham, Franklin, Harnett, Johnston, Orange, Granville, and Wake counties. The Autism Society of North Carolina (ASNC) improves the lives of individuals with autism, supports their families, and educates communities. ASNC is the leading statewide resource organization serving people across the autism spectrum throughout their lifespan. We understand the challenges of the autism community because we work with individuals on the autism spectrum and their families every day.
