AgeOptions is a nonprofit organization connecting older adults and those who care for them with resources and service options so they can live their lives to the fullest. Since 1974, our mission has been to sustain and promote independence for older persons in their own homes and communities. DayOne PACT, formerly PACT, Inc. was founded as a 501(c)(3) non-profit organization in 1981 by families who had relatives with disabilities. Concerned about the future care of their loved ones, they established the agency to assure that there would be guardianship and related services available when they, as parents, were no longer able to function in that capacity. This service was initially provided in Chicago, but later services expanded to include the greater Chicagoland area. PACT, Inc. and DayOne Network merged on January 1, 2016. DayOne Network, founded in 1989 was a "sister" agency to PACT, providing almost the exact services, just in a different location. Providing service coordination to children and adults with developmental disabilities, and their families, DayOne PACT’s Child and Family Connections programs provide services through DuPage, Kane and Kendall Counties. Life Management Services provides trust management, trustee and guardianship services nationwide.

Our group homes provide a family atmosphere where four to eight individuals live together with 24-hour staff support. Each person has their own room, which they are encouraged to personalize. Open Door also utilizes a special dietician-directed meal program called My25. This program considers each person’s health requirements and encourages individuals to help with meal preparation. We also offer assistance for individuals seeking intermittent support. These services are for people who live in their own home or apartment but also need help every now and then. Our staff develops personal relationships with the individuals we serve and offers services in all aspects of everyday living. Open Door is focused on opening doors and developing opportunities for all of our clients. This means that we provide support in every aspect of life, assisting with personal care, doctor appointments, and medication management. Our clients also receive guidance on money management, personal shopping, and laundry assistance. The levels of support are uniquely designed around the specific needs of each person. Fun and recreation are also a big part of services at Open Door. We provide transportation to a wide variety of events and activities. Clients have the opportunity to attend sporting events, see concerts, visit museums, and take outings to local parks. We also have groups that attend dance classes, yoga, and open swim sessions. If an individual has an interest, we try our best to satisfy their need in order to promote independence, growth, and an exceptional quality of life for every person we serve.

Pathlights, formerly known as PLOWS Council on Aging, is a nonprofit that helps navigate and celebrate aging in 20 communities throughout the Southwest Suburbs of Cook County. We help guide adults 60 years of age and over, adults with disabilities, their caregivers, families, and friends along their best path to aging – providing resources, advocacy, programs and services needed to live with independence and dignity in the community. We focus on helping individuals who are navigating the aging process have equal access to food, shelter, care, and safety. Services offered include home-delivered meals and safety checks, in-home services, support groups, education, and Respite for caregivers, protection from abuse, access to benefits for health insurance, Supplemental Nutrition Assistance program (SNAP), energy, transportation, and property tax savings, assistance needed for safe transition from hospitals or skilled nursing facilities, shared housing, and general information about resources.

Guardian Angel Community Services (formerly, Guardian Angel Home), was founded in Joliet, Illinois in 1897 as an orphanage, run by the Sisters of Saint Francis of Mary Immaculate, to care for dependent and neglected children. In 1973, Guardian Angel Community Services became a licensed, child welfare agency and began focusing its efforts on addressing the increasingly complex problems of children and families. In August 2005, the Agency officially changed the name to reflect the wide array of services provided to the community. Guardian Angel Community Services expanded in the mid 1990’s to include an office in Morris, Illinois, which offers counseling, and medical and legal advocacy services to victims of domestic and sexual violence.
