An organizational model that allows not-for-profit hospices (Members) to leverage best practices, achieve economies of scale and collaborate in ways that better prepare each agency to participate in emerging alternative payment models and advance their charitable missions.

Since May 1985, the Brazos Valley Food Bank has been the core of a network of hunger-relief organizations serving the hungry in Brazos Valley. The Food Bank recovers food and grocery product from grocers, USDA and individuals. From 1999 – present, it has increased food distribution from 709,414 to just under 7 million pounds. The Brazos Valley Food Bank strives to alleviate hunger in the Brazos Valley by distributing food and educational resources to our neighbors in need through a network of hunger relief partners. Cy-Hope was birthed in 2011 by a core group of people concerned about the children in our community. CFISD is the third largest school district in Texas, and the largest non-urban school district in the country. CFISD is made up of a majority Latino (45%), Caucasian (24%) and African American (18%) students, with half of the over 117,000 students in CFISD qualifying as economically disadvantaged by federal standards. We have many families and students struggling with hunger, mental health, finishing school, and limited access to enriching experiences like organized sports, college prep or certification classes, private music lessons and summer camp. Cy-Hope strives to fill in these gaps through our programs, partnering with other community organizations to make sure we do not duplicate efforts, but instead collaborate to meet the needs of our community. Family Ties provides Crisis Intervention and Advocacy services to Child Victims and their families as well as Prevention Services to the community. We are a faith-based shelter located in Houston, Texas that empowers, enriches and restores the lives of homeless women and children and provides outreach services to to communities in need, since 1961. The Mission of Yahweh has served homeless women and their children since 1961. The doors are always open for families in crisis. We are a truly unique refuge that not only gives food, clothing and shelter, but also is committed to helping residents to become self-sufficient, productive members of society. The Mission supports more than 2,300 residential stays each month. We also provide thousands of Houston/Carverdale-area residents with donations of food, clothing, school supplies and other necessities.
