As one of the largest waiver providers of its kind in the State of Ohio, Wayside's services include: • 650 People with developmental disabilities supported • 70 Residential Program homes managed • 5 Pathways Adult Day Program sites hosting 250+ participants • 150,000 Medications administered monthly by our Nursing Program • 110 Transportation Program vehicles logging in 1 million miles annually • 40 Communities served in Cuyahoga, Lake, Lorain, and Summit counties • 96,000 “Rides” donated to Wayside over 24 years to raise $29 million • 1,432,952 hours from 400+ highly trained, dedicated staff in 2021 alone

Community Health Services (CHS) is a network of Federally Qualified Health Centers that provide medical, dental and behavioral health care in six counties in northwest Ohio including: Henry, Hancock, Allen, Huron, Wood and Sandusky. CHS operates nine medical offices in the region and our main campus is based in Fremont, Ohio. CHS providers offer a broad spectrum of care in specialties such as Family Practice, Pediatrics, Internal Medicine, Obstetrics, Gynecology, Behavioral Health and Dentistry. CHS is accredited by the Joint Commission and is certified as a “Primary Care Medical Home” or PCMH. This special accreditation means that CHS is recognized as a model for primary care excellence that is patient-centered, comprehensive, coordinated, accessible, and focused on quality and safety. The Richland County Foundation is a 501(c)(3) nonprofit organization, incorporated in 1945. It was founded by local citizens dedicated to serving the public good in our community. Richland County Foundation is the one of the largest community foundations in the state of Ohio and the largest private source of grants in Richland County. Grants are awarded for a broad range of community needs as well as college scholarships to undergraduate students. The Foundation is governed by a volunteer board of trustees.
