-Develops and maintains an Individualized Service Plan with a client, their family, and other team members. -Coordinates community resources, programs, and agencies that are funded by the Division of Developmental Disabilities -Utilizes natural supports through the client's neighborhood -Reviews various assessments and evaluations -Schedules and facilitates planning team meetings with the client-Writes Person Centered Planning Tool and Individualized Service Plan -Distributes Individualized Service Plan and Person Centered Planning Tool to the client, client's family, team members, and service providers -Monitors and reviews Individualized Service Plan in intervals -Contacts clients on a monthly basis. Face to face visits on a quarterly basis and home visits on an annual visit to review Individualized Service Plan and documents in the monitoring tool.
