AgeSpan, formerly Elder Services of the Merrimack Valley and North Shore, is a private nonprofit agency established in 1974 to help people in northeast Massachusetts maintain their highest levels of independence, health, and safety as they age. AgeSpan serves as a trusted resource, connecting people of all ages and abilities and their caregivers with impartial information, quality services, and vigorous advocacy to help them lead fulfilling lives in the community. AgeSpan is a designated federal Area Agency on Aging (AAA), a Massachusetts Aging Service Access Point (ASAP), and elder protective service agency.

Peace Care is a not for profit organization sponsored by the Sisters of St. Joseph of Peace, where we strive to provide high-quality faith-based care to the members of our Jersey City community. Southern New Hampshire Services, Inc. (SNHS) is a private non-profit corporation chartered in the State of New Hampshire, May 21, 1965 to serve as the Community Action Agency for Hillsborough County in compliance with the Economic Opportunity Act of 1964. From 1965 through 1969, SNHS was known as the Community Action Agency for Hillsborough County and served the City of Nashua and the twenty-nine towns. In 1969 the organization became the Community Action Agency for the City of Manchester as well. In 1974 the agency’s name was changed to Southern New Hampshire Services, Inc. In July, 2011, Rockingham Community Action (RCA), the Community Action Agency serving Rockingham County, was merged with Southern New Hampshire Services. SNHS had served as the administrative agent for RCA since October, 2008. As a result of this merger SNHS now provides services to residents of both Hillsborough and Rockingham counties. As a Community Action Agency, it is SNHS’ purpose to provide social service programs and advocacy activities for the economically disadvantaged, elderly, youth and other specialized populations in the organization’s service area. SNHS operates 64 programs, contracting with the Federal, State and local governments as well as other social service agencies and private entities. The programs offered include Child Care, Early Head Start, Head Start, Women Infants and Children (WIC) Nutrition Program, Commodity Supplemental Food Program (CSFP), Energy Assistance, Weatherization, Asset Development, Supportive Housing for the Homeless, Homelessness Prevention programs, Retired Senior Volunteer Program (RSVP), as well as several initiatives under the Workforce Investment Act. For elders and younger disabled individuals who are frail, being in their own home provides security and a very important sense of independence. Elder Services of Worcester Area, Inc.'s mission is to help Elders, Disabled Individuals, and their Caregivers obtain essential services so that they can remain at home with dignity and independence for as long as safely possible. Elder Services of Worcester Area, Inc. is a private, not-for-profit agency and is one of twenty-seven Aging Services Access Points in Massachusetts. Building on more than two decades of experience, our organization is an expert in community based, long term care, assisting elders over the age of sixty and younger disabled individuals in the Greater Worcester area since 1974. Harbor Care - formerly known as Harbor Care, Keystone Hall, Southern NH HIV/AIDS Task Force, Healthy at Home, and the Partnership for Successful Living - is an innovative nonprofit community organization helping individuals and families of New Hampshire find solutions to many of life’s most challenging issues. We provide high quality housing and residential services, primary and behavioral health care, substance use disorder treatment, home care, HIV/AIDS care, veteran services, and supports to individuals and families who are experiencing or at risk of homelessness. Our unique model of “housing + integrated care = success” allows us to provide the wide range of collaborative services people need to gain and sustain independence. Each year, Harbor Care provides integrated care and housing to 5,000+ New Hampshire community members statewide, with a focus on the Greater Nashua, Manchester, Salem, Antrim, Plymouth, Claremont, Keene, and southern New Hampshire regions. The Community Action Program Belknap-Merrimack Counties, Inc. (CAPBMCI, CAPBM, CAP-BM, BMCAP) is a New Hampshire-based 501(c)(3) private, nonprofit organization. The agency was established in 1965 under the provisions of the Equal Opportunity Act of 1964. Under this Act, over twelve hundred Community Action Agencies (CAA) were established nationwide. The primary mission of the organization is to work with low-income families, the elderly, and individuals with disabilities to assist them in their efforts to become or remain financially and socially independent. The Agency accomplished this task by providing a broad array of services that are locally defined, planned, and managed. The agency sponsors and manages seventy-three (73) programs designed to meet the needs of children, families, single parents, and the elderly. Without the services provided by our agency, many local residents would be without a means to provide for their basic needs, including food and shelter. The agency is funded by Federal, State, county, and local funds (37 cities and towns in Belknap and Merrimack Counties). The agency receives United Way grants, foundation and charitable grant funds, fees for service, private business donations, and donations from individuals.
