Service Coordination, Inc. is a private, nonprofit organization providing case management services throughout the state of Maryland to individuals with disabilities, those with complex medical needs and serious mental health disorders, transitioning youth, court-involved individuals, older adults and veterans. We believe that services should be tailored to meet people’s needs. Service Coordination provides support to adults and children by connecting them with needed resources and services in their communities. With over 850 team members in 2 offices in 14 counties plus Baltimore City, Service Coordination serves more than 14,000 people around the state.

CSS provides quality, community-based services to individuals with developmental disabilities. Individuals are supported in accordance with their needs in school, work, home, and the community. Services are designed by the individual, the individual's family, and others who know the individual well. Services are rendered in the environments in which the individual desires to live, work, recreate, or pursue educational goals, and are designed to support each to pursue his or her own self-directed life goals.

The Arc of Frederick County provides advocacy, access to resources, and assistance for increasing individual and family connections for people with developmental disabilities. The organization seeks creative solutions to help people with developmental disabilities attain their goals.

The Arc Montgomery County connects people of all ages and abilities with their communities to build inclusive and fulfilling lives. We support children, youth, adults and seniors, with and without disabilities, and their families by providing inclusive child care, before and after school youth programs, work skills training, employment, community living support, resources, advocacy support and respite care. People with intellectual disabilities have a safe place to live at CPARC, and services that encourage them to grow. “CPARC helps us grow and grow and grow, like a flower,” says Bryan, one of the folks we support. Just 60 years ago, people with disabilities were living in institutional settings. At that time, the only option for parents of a child with a disability was to place their child in a state institution. A group of concerned parents created CPARC in 1953 to offer an alternative to institutionalization for their children with special needs. Bar-T provides care and innovative programs for over 2,500 children within 30 before and aftercare centers, 6 preschool classrooms, an outdoor education facility, and two Early Childhood programs in Montgomery County, MD. Unified Community Connections is a private, nonprofit organization that serves, assists and advocates for children and adults with cerebral palsy and other physical disabilities. Headquartered in Hunt Valley, Md., Unified Community Connections has four satellite centers throughout Maryland, enabling the organization to provide programs and services throughout the state.

For nearly 50 years, Cornerstones has been helping neighbors in need overcome tough economic times in an already high cost-of-living region. The service area of Cornerstones has grown to include most of northwestern Fairfax County. Each of us can become a Cornerstone and continue our history of helping people build more stable lives by connecting them to vital resources that solve their needs for housing, childcare, food or financial assistance.

Since 1992, NCHH has served as a highly regarded and credible change agent, successfully integrating healthy housing program and capacity building, research, and advocacy under one roof to reduce health disparities and improve housing quality nationwide. Program and Capacity Building: NCHH is a valuable resource to federal, state, and local agencies due to its unique ability to identify and convert credible science, technical expertise, and over 20 years of practical experience into accessible and useful information for dissemination, local adaptation, and program growth. NCHH uses this wealth of knowledge and skill to provide technical assistance, strategic planning, and program evaluation for a variety of program innovation and planning tasks. Relevant examples of our work include leading the Grassroots Advocacy Network to develop and test local policies and solutions for healthy housing and developing a Healthcare Financing Resource Library, which identifies strategies to finance healthy homes services through the healthcare system. Research: NCHH designs research and evaluation activities with both policy and practice in mind and translates credible, science-based research into practical solutions. Previous efforts have provided valuable insights into topics such as the effectiveness of lead hazard control interventions and the impact of green housing on occupant health. Current research includes studies that evaluate the health benefits of green affordable housing and improve the quality of life of low-income older adults so that they may safely remain in their homes. Advocacy: NCHH’s advocacy efforts provide expertise, facilitation, and support to government agencies, legislative bodies, and advocates as they establish policies to create healthier homes through laws and regulations. Promotion of the National Healthy Housing Standard, which is intended to eliminate hazardous conditions in existing housing, is a current example of our work.
