Suburban Access provides individual service coordination and case management for individuals with intellectual disabilities in south and west suburban Cook County, Illinois. Programs include the adult DD program, Child and Family Connections Early Intervention, and Early Intervention monitoring.

Cornerstone Services provides progressive, comprehensive services to people with disabilities promoting choice, dignity, and the opportunity to live and work in the community. Since 1969 Cornerstone has been a leader in meeting the unique needs of individuals with developmental disabilities, mental illnesses, physical disabilities and sensory impairments. Cornerstone is licensed by state and federal agencies. We are an Illinois-based, not-for-profit organization which provides employment, community support, residential and behavioral health services through four program departments.

Our group homes provide a family atmosphere where four to eight individuals live together with 24-hour staff support. Each person has their own room, which they are encouraged to personalize. Open Door also utilizes a special dietician-directed meal program called My25. This program considers each person’s health requirements and encourages individuals to help with meal preparation. We also offer assistance for individuals seeking intermittent support. These services are for people who live in their own home or apartment but also need help every now and then. Our staff develops personal relationships with the individuals we serve and offers services in all aspects of everyday living. Open Door is focused on opening doors and developing opportunities for all of our clients. This means that we provide support in every aspect of life, assisting with personal care, doctor appointments, and medication management. Our clients also receive guidance on money management, personal shopping, and laundry assistance. The levels of support are uniquely designed around the specific needs of each person. Fun and recreation are also a big part of services at Open Door. We provide transportation to a wide variety of events and activities. Clients have the opportunity to attend sporting events, see concerts, visit museums, and take outings to local parks. We also have groups that attend dance classes, yoga, and open swim sessions. If an individual has an interest, we try our best to satisfy their need in order to promote independence, growth, and an exceptional quality of life for every person we serve.

Individual Advocacy Group is a CARF accredited not-for-profit organization dedicated to community- based supports and personal advocacy for individuals with special needs. IAG provides training programs, behavioral and individual therapies, community- based residential programs, advocacy and case management for individuals who have intellectual or developmental disabilities, mental health disorders, brain injuries or physical/medical conditions. All of IAG’s programs are community- based and all residential programs are family scale of four or fewer people using neighborhood housing and apartments. IAG provides its services in 21 counties in Illinois from Chicago to the Quad Cities to Springfield, as well as the District of Columbia and Maryland. IAG believes that all individuals deserve and should receive respect regardless of their history, type of disability or severity of specialized needs. Furthermore, all individuals must be assured of their endowed rights of community participation and personal independence shared by all members of society. Each individual can meet his or her personal goals, follow personal interests, and find personal success and happiness. Accordingly, people with disabilities can achieve personal success and happiness if given the chance and the support. Innovations In Learning, PC, is a multi-faceted agency. We provide intensive ABA services to children with Autism and related disorders and Behavior Management therapy to individuals in the Waiver program, Day Services for adults with disabilities as well as outpatient services for children and adults including individual and group therapy. In all areas of our work we take pride in using empirically based interventions. We develop objective and measurable goals that allow close monitoring of treatment efficacy. Meals on Wheels began service in 1977 in southern Lake County, Indiana as part of the Visiting Nurse Association. At that time, meals were obtained from hospitals and nursing homes and delivered to individuals in their home. As our program continued to grow, it was necessary to open a central kitchen to prepare meals for our clients. In 1993, we purchased a building in Crown Point . We outgrew the facility in 2002 when our meal count numbers grew to over 1,200 meals daily for clients living throughout Lake County and in 2006 moved to our current location.
