Suburban Access provides individual service coordination and case management for individuals with intellectual disabilities in south and west suburban Cook County, Illinois. Programs include the adult DD program, Child and Family Connections Early Intervention, and Early Intervention monitoring.

Our group homes provide a family atmosphere where four to eight individuals live together with 24-hour staff support. Each person has their own room, which they are encouraged to personalize. Open Door also utilizes a special dietician-directed meal program called My25. This program considers each person’s health requirements and encourages individuals to help with meal preparation. We also offer assistance for individuals seeking intermittent support. These services are for people who live in their own home or apartment but also need help every now and then. Our staff develops personal relationships with the individuals we serve and offers services in all aspects of everyday living. Open Door is focused on opening doors and developing opportunities for all of our clients. This means that we provide support in every aspect of life, assisting with personal care, doctor appointments, and medication management. Our clients also receive guidance on money management, personal shopping, and laundry assistance. The levels of support are uniquely designed around the specific needs of each person. Fun and recreation are also a big part of services at Open Door. We provide transportation to a wide variety of events and activities. Clients have the opportunity to attend sporting events, see concerts, visit museums, and take outings to local parks. We also have groups that attend dance classes, yoga, and open swim sessions. If an individual has an interest, we try our best to satisfy their need in order to promote independence, growth, and an exceptional quality of life for every person we serve.

Cornerstone Services provides progressive, comprehensive services to people with disabilities promoting choice, dignity, and the opportunity to live and work in the community. Since 1969 Cornerstone has been a leader in meeting the unique needs of individuals with developmental disabilities, mental illnesses, physical disabilities and sensory impairments. Individual Advocacy Group is a CARF accredited not-for-profit organization dedicated to community- based supports and personal advocacy for individuals with special needs. IAG provides training programs, behavioral and individual therapies, community- based residential programs, advocacy and case management for individuals who have intellectual or developmental disabilities, mental health disorders, brain injuries or physical/medical conditions. Pathlights, formerly known as PLOWS Council on Aging, is a nonprofit that helps navigate and celebrate aging in 20 communities throughout the Southwest Suburbs of Cook County. We help guide adults 60 years of age and over, adults with disabilities, their caregivers, families, and friends along their best path to aging – providing resources, advocacy, programs and services needed to live with independence and dignity in the community. We focus on helping individuals who are navigating the aging process have equal access to food, shelter, care, and safety. Services offered include home-delivered meals and safety checks, in-home services, support groups, education, and Respite for caregivers, protection from abuse, access to benefits for health insurance, Supplemental Nutrition Assistance program (SNAP), energy, transportation, and property tax savings, assistance needed for safe transition from hospitals or skilled nursing facilities, shared housing, and general information about resources.

Step By Step Inc. has been providing quality Early Care & Education programs for children 6 weeks to 11 years old since 1993. Our mission is to provide every child an opportunity to grow and learn in a warm and loving environment. Our six programs are fully licensed as Day Care Centers by the Illinois Dept. of Children and Family Services. Our Alton and Brighton locations offer a part-day Illinois State Board of Education Pre-K program for eligible children. All of our centers also provide a USDA approved food program for all children enrolled.
