Every warrior has a next mission. We know that the transition to civilian life is a journey. And for every warrior, family member, and caregiver, that journey looks different. Wounded Warrior Project (WWP) began in 2003 as a small, grassroots effort providing simple care and comfort items to the hospital bedsides of the first wounded service members returning home from the conflicts in Iraq and Afghanistan. As their post-service needs evolved, so have our programs and services. Today, through our direct programs in mental health, career counseling, and long-term rehabilitative care, along with our advocacy efforts, we improve the lives of millions of warriors and their families. This effort requires the passion and commitment of friends, supporters, and like-minded organizations that enable us to fulfill our mission. With their collaboration and support, we strive to ensure that when those who serve come home, they’re afforded every opportunity to be as successful as a civilian as they were in the military.

Alivia (alleve- ē- uh) Care, Inc., is a 501c3 based in Jacksonville, Florida whose mission is to provide leadership support to create innovative care solutions through best in class services, collaborative partnerships, and strategic investments. After a multi-year strategic planning effort focused on anticipated changes in the care of patients with advanced illness, the Board and executive leadership of Community Hospice & Palliative Care formed Alivia Care Inc. in 2020. The strategic intent of Alivia Care, Inc. is to provide a structure that will support the expanded mission and vision of Community Hospice & Palliative Care and prepare it to respond to changes in care delivery and reimbursement for patients living with advanced or chronic illnesses. The structure of Alivia Care was also designed to support additional programs and services beyond hospice care that will fulfill its goal of serving more people in more places, in order to help them live better with advancing illness. Meridian, a private, non-profit organization, has its roots in the community mental health movement that began in the 1960’s to bring education about mental illnesses and substance use disorders and treatment for those affected to the local level. Created as a tax-exempt entity in 1972, Meridian has been a part of the lives of thousands as we provide a safety net for those in crisis, a source of effective treatment, and a contributor to the health of the entire community. Our mission – To promote the health, recovery, and well-being of those affected by mental illness and substance use disorders through prevention, coordinated treatment, and supportive services – is evident in all we do; from developing a continuum of treatment services that are evidence-based for a range of illnesses to participating in our communities as a partner in enhancing the quality of life. Community Partnership for Children cares for over 1,400 children each year who are victims of abuse or neglect. We serve families and children in Flagler, Putnam, and Volusia Counties in Florida. We partner with providers and local citizens to restore families, support caring relatives, connect children with loving homes, and prepare adolescents for adulthood. Because every child deserves a loving home, Community Partnership for Children coordinates preventative and early intervention services that enable children to remain with their families. When a child must be removed from the family, we strive to maintain them in the least restrictive environment available, usually with a relative or in a foster home, until the child can be reunited with their family. Community Partnership for Children promotes sibling unification which enables brothers and sisters to share their lives together while in foster care. Community Partnership for Children works with many local providers to provide child protection. Our system-of-care overview shows the relationship between our partners. Through the efforts and involvement of the community, we are able to keep children safe and build strong families.

Daniel is Florida’s oldest nonprofit child-service agency. The organization was originally established as an orphanage in 1884 as a living tribute to James Jaquelin Daniel, the legendary Jacksonville community leader who dedicated his time, wealth and life to the betterment of children. Daniel has since evolved into a multi-service agency that assists an average of 3,000 children, adolescents and families each day through a variety of innovative and nationally recognized programs. In 1976 Hubbard House opened its doors as the first domestic violence shelter for victims and their children in Florida. In the past 45 years, Hubbard House has evolved into a nationally recognized full-service, certified domestic violence center providing programs and services to more than 4,000 women, children, and men annually in Duval and Baker counties. As one of the first organizations in Florida to serve children with special needs, Pine Castle helped pioneer the philosophy that individuals with intellectual/developmental (I/D) differences have the right to live whole and productive lives. Pine Castle was founded in 1952 by a group of 33 families of children with I/D differences. No local programs or schools existed at that time to provide support services, except for a few distant residential institutions run by the state. Parents were forced to choose between institutionalization or keeping their children at home. In 1965, the public school system initiated a program for students with special needs. At that time, Pine Castle transitioned to a work and life skills training and activity center for adults with differences. Today, Pine Castle is a leader in serving adults with intellectual and developmental differences and providing support to their families across Northeast Florida. More than 300 clients come through our programs daily, learning valuable skills. Pine Castle offers many opportunities for adults with intellectual and developmental differences, from basic academic training and hands-on job experience to leisure activities and life skills coaching. In 1964 President Lyndon B. Johnson declared an "unconditional war on poverty" and created the 1964 Economic Opportunity Act. Working with the poverty task force created by Kennedy, President Johnson created the Office of Economic Opportunity to administer a wide variety of poverty prevention and alleviation programs created under the 1964 Economic Opportunity Act. Community Action Agencies were founded on the idea that local citizens were the most knowledgeable and best equipped to address problems of poverty in their communities. Community Action gave previously powerless people the means to work toward their own economic and social advancement. Community Action Agencies help people to help themselves in achieving self-sufficiency. Today, there are approximately 1,000 Community Action Agencies in the United States.
