On Lok is a nonprofit organization that empowers older adults to age with dignity and independence. With over 50 years of experience, On Lok is a trusted resource in San Francisco, Santa Clara County, and the Tri-City area of Alameda County. Our mission: to relentlessly pursue quality of life and quality of care for older adults and their families. On Lok founded and still operates the Program of All-Inclusive Care for the Elderly (PACE), which allows eligible seniors to live at home for as long as possible by providing comprehensive medical care and social services. On Lok leverages its unique expertise as the pioneers of PACE through On Lok PACEpartners, a consulting service that supports organizations interested in exploring PACE, developing a PACE program, or improving their PACE outcomes. On Lok also operates On Lok 30th Street Senior Center, the largest multipurpose senior center in San Francisco, which provides a second home, health programs, activities, and case management services to active seniors. While the On Lok Mission Nutrition Program, provides more than 250,000 meals annually to seniors through its dining locations and meal delivery program. In 2021, Openhouse and On Lok joined forces to create Openhouse + On Lok Community Day Services, a safe and life-affirming space for LGTBQ+ seniors to maintain wellness and empower independence.

Golden Gate Regional Center is a state-funded nonprofit organization serving individuals with developmental disabilities in Marin, San Francisco and San Mateo counties. We assist individuals and their families in securing services to enable them to lead more independent, productive and normal lives. The regional center system was established in California through legislation supported primarily by Assembly member Frank D. Lanterman. Its origins, however, lay in the California State Legislature's authorization of pilot regional centers in the San Francisco Bay Area (what is now GGRC) and Los Angeles County (now Lanterman Regional Center). The purpose of these early regional centers, which began operating in 1966, was to assist persons with intellectual disabilities and their families in locating and developing services for their special needs. Following the establishment of the pilot projects, which had promising early results, the Legislature passed the Lanterman Mental Retardation Services Act. Enacted in 1969, the legislation established a statewide system consisting of the two original regional centers plus five more, each serving a distinct geographical area. (Subsequent legislation has grown the number of regional centers to 21.) A second Lanterman act, the Lanterman Developmental Disabilities Services Act, was made law in 1974. It expanded the regional centers'​ service population to include persons with cerebral palsy, epilepsy, autism and other disabling conditions found to be closely related to intellectual disability. In 1993, the California Early Intervention Services Act expanded the regional centers'​ purview once more, this time to include early intervention ("Early Start"​) services for infants and toddlers (up to 36 months of age) who have a developmental delay or are at high risk of developing a delay. Canine Companions is a national nonprofit organization that enhances the lives of people with disabilities by providing expertly trained service dogs and facility dogs – all at no cost to our clients. Established in 1975, Canine Companions headquarters is located in Santa Rosa, California. We have six training centers across the country: in Northern California, Southern California, Florida, Ohio, New York and Texas. We are recognized worldwide for the excellence of our dogs and the quality and longevity of the matches we make between our dogs and clients. All training and ongoing follow-up services are also provided free of charge.

The Mission of the Chinatown Community Development Center is to build community and enhance the quality of life for San Francisco residents. We are a place-based community development organization serving primarily the Chinatown neighborhood, and also serve other areas including North Beach and the Tenderloin. Brilliant Corners provides innovative housing and housing-related services to California’s most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. We were founded in 2004 on the belief that all people deserve safe, affordable housing that includes the support services they need to live as independently as possible. We quickly became California’s largest housing partner in the historic shift from institutional settings to community-based housing for individuals with intellectual and developmental disabilities (I/DD). Over the past 15 years, we have embraced the challenge of scaling innovative supportive housing for those who need it most, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. Working at the intersection of housing and sectors such as healthcare, veterans’ services, criminal justice, and the homelessness response system, Brilliant Corners provides a variety of innovative supportive housing models that meet the unique needs of each individual we serve. Brilliant Corners’ approach is based on Housing First principles, person-centered planning, and community living in the least restrictive setting. Our staff work hand-in-hand with our clients to achieve their self-identified goals—including safe housing, financial stability, improved medical and mental health, connecting with family, community engagement and an overall improved quality of life. Self-Help for the Elderly is a private community-based non-profit organization. Our mission is to promote the independence, dignity and self-worth of seniors. We will achieve this by empowering seniors to help themselves and by providing a comprehensive range of multicultural and multilingual services. Founded in 1966, Self-Help offers a vast array of health care and social services to over 40,000 seniors and disabled adults annually in San Francisco, San Mateo, Santa Clara and Alameda counties. More than 90% of the agency’s clients are members of ethnic communities; and over 90% of Self-Help’s current staff is bilingual and bicultural in various Chinese dialects, Vietnamese, Tagalog, Spanish or Russian. All direct service staff are bilingual in at least one of the above languages and are culturally and linguistically competent to provide services to the target population. Self-Help for the Elderly has earned a reputation for employing and providing housekeeping jobs in San Francisco in the form of house cleaning services, commercial cleaning services, and job training programs. In addition, current customers have come to rely on their social services such as home health care, assisted living, hospice care, in-home care, home care assistance, senior care, adult day care, and elderly care.
