Jewish Family Service (JFS) is a non-profit home health and social service agency providing a broad continuum of services including skilled home health care, private duty in-home care, palliative care, counseling, care management, guardianship/conservatorship, meals on wheels, transportation for older adults, food and financial assistance, adoption services and services to the developmentally disabled and chronically mentally ill. We serve over 3,000 individuals annually throughout Hampton Roads, and beyond, and have a strong volunteer program. Our mission is to provide a full spectrum of services that promote quality of life within the philosophy and tradition of Judaism. JFS's skilled home health agency is Medicare-certified and accredited by the Community Health Accreditation Partner (CHAP). Our patient outcomes and satisfaction results consistently exceed state and national benchmarks. In 2012, JFS, along with Beth Sholom Village, established the Freda H. Gordon Hospice and Palliative Care of Tidewater, an organization accredited by the Joint Commission and Medicare-certified providing hospice care throughout our community. Founded in 1973, Senior Connections is a nonprofit agency with a mission to provide essential home and community-based care to maximize independence for seniors. We provide home delivered meals, in-home care, home repair and operate DeKalb County Senior Centers with an emphasis on health and wellness. Funding for our services comes from a variety of sources: contract for services; private donations; grants; proceeds from our Connections Catering. All services are available free of charge (if qualified); partial pay; or private pay. We produce more than 3,000 meals each day deliver meals throughout metro Atlanta and middle Georgia. We provide more than 50,000 hours of in-home care annually and pump millions back into communities through senior home repairs.

The Hampton Roads Community Action Program, formerly known as the Office of Human Affairs, was founded in 1966 as the Newport News Office of Economic Opportunity as a direct result of President Lyndon Johnson’s War on Poverty. As the 501(c)(3) Community Action Agency, first for Newport News and later expanding into Hampton, HRCAP has worked for over 50 years to promote the self-sufficiency of the low-income residents living in our service area and ameliorate the brutal impact of poverty on the lives of their children. Starting with a handful of initiatives (Medicare Alert, Neighborhood Youth Core, Head Start) HRCAP grew in the 70’s and 80’s to include an ever expanding range of services to low-income residents. Shifts in federal, state and local funding priorities often threatened the agency, an occasionally resulted in the reduction or elimination of programs, but HRCAP persevered and continues to offer a wide range of needed services to the community. The Hampton Roads Community Action Program's mission is “to plan, develop and implement programs that foster self-sufficiency through educational, social, physical and economic development.” The agency operates over 20 programs designed to fulfill that mission within the community. Programs include early childhood education (Head Start, Four-Year-Old Initiative); housing (Fair Housing, Housing Counseling); counseling for pregnant teens (Teen Moms Support Program); assistance to the homeless (Clean Comfort); nutrition services (Summer Lunch); emergency assistance (Emergency Food and Shelter); affordable housing and community development (Peninsula Community Development Corporation) and more.

Professional Rehabilitation Consultants (PRC) is an outpatient occupational therapy clinic specializing in Mental Health. PRC is medicare-certified and accepts most insurance plans. Using skilled, evidence-based approaches, our team of occupational therapists (OT) treats a variety of diagnoses including depression, bipolar, anxiety, personality disorders, developmental disorders, chemical dependency, and more. Additionally, occupational therapists provide interventions for the management of chronic and acute medical conditions. PRC offers Buried in Treasures, a hoarding program, facilitated by OTs with experience in both mental health and excessive acquiring. Personalized treatment plans are developed for each client with the common goal of improving their ability to participate and find purpose in daily occupations. The PRC team reinforces the individuality of mental wellness. No one person's health will look or feel the same, which is one of the many reasons why the broad scope of OT is well suited in mental health. We utilize a 'library' of skilled approaches and adapt and modify them based on our clients' specific needs. Our interventions are developed in consideration of each clients' physical, cognitive and emotional functioning in order to promote success and increased independence in day-to-day activities. Frequently addressed areas of need include mental health and symptom management, self-care habits and routines (dressing, bathing, oral cares, sleep hygiene, etc.), independent living skills, medication management, community reintegration, improving structure of time, cognition, self-regulation, communication / interpersonal skills, relapse prevention, home management, and job readiness/volunteering.
