Family Circle of Care is a health system that operates six medical clinics, one dental clinic, and a mobile medical clinic in the East Texas cities of Tyler, Athens, and Jacksonville and surrounding communities. We provide Obstetrics & Gynecology, Pediatrics, Family Medicine, Internal Medicine, Mental Health Services, Dental Services, and Social Services and Resource Assistance, and have been the dominant community health center, (also known as a Federally Qualified Health Center or FQHC) in our service area since 2013. FCC’s clinics are in tracts designated as Opportunity Zones and Medically Underserved Areas, with low median household incomes, high unemployment rates, high percentages of poverty, low percentages of residents with an advanced degree, and high percentages of students eligible for free or reduced lunch. Our five Tyler clinics are in non-rural areas and our Jacksonville and Athens clinics are in rural areas. Many of our patients experience barriers to healthcare, and we strive to reduce obstacles to provide trauma-informed, culturally competent, affordable, accessible healthcare for all. Precision Spine Care combines the expertise of fellowship-trained spine surgeons, non-surgical spine MDs, interventional pain specialists, and physician assistants experienced in spine and on-the-job injury. The spine center assembles all these spine care specialists, along with X-ray, MRI diagnostics, an injection suite, and an ambulatory surgery center — all under one roof for the convenience of those with back and neck pain. For patients outside the Tyler area, Precision Spine Care offers quality spine care and pain management at multiple satellite locations across East Texas, including, Longview, Texarkana, Nacogdoches, Lufkin, Dallas, Addison, Rockwall, and Forney.

SHR emerged from two programs joined in an effort to serve the rapidly growing HIV population of East Texas in 1989. What began two decades ago as an effort to serve that emerging HIV population of East Texas, has now expanded into full service clinics catering to the most under-served in our communities. SHR offers a wide variety of services to our patients with the goal of treating the whole patient and the whole family under one roof.
