The Ohio Department of Aging works to ensure that our elders are respected as vital members of society who continue to grow, thrive and contribute. We collaborate with state agencies and community partners, including area agencies on aging, to help integrate aging needs into local plans and ensure that aging Ohioans have access to a wide array of high-quality services and supports that are person-centered in policy and practice. Community Health Services (CHS) is a network of Federally Qualified Health Centers that provide medical, dental and behavioral health care in six counties in northwest Ohio including: Henry, Hancock, Allen, Huron, Wood and Sandusky. CHS operates nine medical offices in the region and our main campus is based in Fremont, Ohio. CHS providers offer a broad spectrum of care in specialties such as Family Practice, Pediatrics, Internal Medicine, Obstetrics, Gynecology, Behavioral Health and Dentistry. CHS is accredited by the Joint Commission and is certified as a “Primary Care Medical Home” or PCMH. This special accreditation means that CHS is recognized as a model for primary care excellence that is patient-centered, comprehensive, coordinated, accessible, and focused on quality and safety.
