We collaborate with state agencies and community partners, including area agencies on aging, to help integrate aging needs into local plans and ensure that aging Ohioans have access to a wide array of high-quality services and supports that are person-centered in policy and practice. Our programs include the PASSPORT Medicaid waiver, caregiver support, the long-term care ombudsman program, the Golden Buckeye Card and more. Our ultimate goal is to position Ohio on the leading edge of innovation and responsiveness to the growing and changing aging population. Community Health Services (CHS) is a network of Federally Qualified Health Centers that provide medical, dental and behavioral health care in six counties in northwest Ohio including: Henry, Hancock, Allen, Huron, Wood and Sandusky. CHS operates nine medical offices in the region and our main campus is based in Fremont, Ohio. CHS providers offer a broad spectrum of care in specialties such as Family Practice, Pediatrics, Internal Medicine, Obstetrics, Gynecology, Behavioral Health and Dentistry. CHS is accredited by the Joint Commission and is certified as a “Primary Care Medical Home” or PCMH. This special accreditation means that CHS is recognized as a model for primary care excellence that is patient-centered, comprehensive, coordinated, accessible, and focused on quality and safety. Patients are treated with respect, dignity and compassion. Over the years, CHS has earned the reputation as a leader for excellence in the region and has a proven track record of meeting or exceeding standards set for the health care industry. Dedicated to meeting the needs of typically rural and under-served communities, income is never a barrier to care at CHS. CHS accepts Medicaid, all Medicaid managed care plans, Medicare, and most insurance companies. Self-pay patients are also accepted and a sliding fee schedule is available for qualifying patients based on income and family size. CHS actively supports and participates in collaborative programs designed to enhance access and quality of health care in the communities that we serve. Partnering with local and regional hospitals, health departments, agencies and other organizations, CHS continues to seek out innovative strategies to maximize the impact local and regional health care has on the health and wellness of the community.

Toledo Community Foundation provides grants and support to nonprofit organizations and scholarships to individuals. These services are supported by partnerships with private foundations and more than 900 individual funds representing a unique charitable giving partnership with an individual, family, business or nonprofit organization.

The Hospital Council of Northwest Ohio (HCNO) is dedicated to working in partnership to improve the health of the region's communities. HCNO is a member-driven organization that represents and advocates on behalf of its member hospitals and health systems. It also provides collaborative opportunities to enhance the health status of residents in the region. HCNO coordinates community health improvement efforts, including the Northwest Ohio Pathways HUB and Healthy Lucas County coalition. Additionally, HCNO coordinates disaster preparedness and community health assessments in the region and beyond.

Marion Goodwill Industries, Inc. is one of over 156 independent Goodwill agencies. All with a common goal to help unemployed individuals overcome their barriers to employment. Founded in 1977, Marion Goodwill has expanded from one retail store in downtown Marion to 300 employees working in 14 locations throughout our 5 county territory in Ohio. Our service territory includes: Marion, Delaware, Union, Crawford and Morrow Counties. Our mission is to provide training, employment and support services for people with disabilities or disadvantages who seek greater independence. Employment and training are Goodwill's primary focus, assisting men and women to develop their job skills and secure work opportunities that fulfill their personal and professional goals.
