Grace Health is a Federally Qualified Health Center that has been providing health care in Calhoun County since 1986. Our services include: Family Practice, Internal Medicine, Pediatrics, OB/GYN, Dental, Behavioral Health, Optometry/Vision Care, Podiatry and Physical Therapy. Grace Health accepts Medicaid, Medicare, uninsured and most commercial insurances. Currently, over 300 staff are employed by Grace Health. Founded by Community Action Agency in 1991, the Center for Healthy Beginnings was a prenatal clinic. Today, it has morphed into the Center for Family Health, providing primary health-care services, including prenatal, pediatric, adolescent, adult, geriatric, behavioral health and dental care. The Center is one of 1,400 community health centers across the nation; they account for 8,000 sites and serve more than 25 million patients. The Center for Family Health is a Federally Qualified Health Center (FQHC) located in downtown Jackson, Michigan. Our mission is “Opening the Door to Health Care for All”, which means we turn no patient away based on his or her ability to pay. We provide top-notch primary care services, including Family Medicine and Pediatrics. In addition, we provide Dental, Behavioral Health, and supportive services including Pharmacy, Case Management, Nutrition Counseling, Transportation, Interpretation, and Enrollment Services. Our physicians and midlevel providers have extensive training and experience in patient care.

Our program coordinates and provides care for the at-risk frail elderly population of our community through an integrated team of health care professionals made up of medical doctors, physician assistants, nurse practitioners, physical therapists, occupational therapists, speech therapists, recreational therapists, dieticians, and medical social workers all under one roof. This team meets daily to assess the complex health and social needs of our participants and to make any necessary adjustments to the personalized care plan for each individual. Additional medical specialties such as audiology, dentistry, optometry, and podiatry are also offered. Our in-house day center, medical clinic and therapy rooms make it highly convenient for our participants to receive the socialization they need as well as receive comprehensive medical care under one roof.

Careline Health Group is a value-based alliance that provides Hospice care and Physician Services for the frail or people facing terminal illness. Our Mission: To Serve Together, Provide Value and Deliver Exceptional Care
