Thundermist Health Center is a non-profit community health center serving three communities. We serve Woonsocket, South County (formerly the Health Center of South County), and West Warwick. Thundermist provides comprehensive health care services in order to meet the needs of each community....This includes: Primary medical care, by board-certified, full-time staff physicians who carry full hospital admitting privileges in our communities of service. Dental care provided by dentists and dental hygienists in our health centers and mental health services in a fully integrated model alongside our medical services. Plus, pharmacy services, through a partnership with CVS/pharmacy, allows patients to take advantage of Section 340B special pricing afforded by the US Public Health Services. CVS pharmacy offers dramatically reduced pricing to our patients at our in-house pharmacy in Woonsocket and at CVS pharmacies located in Wakefield and West Warwick. We also maintain a team of social workers who help patients to overcome the social barriers that keep them from good health and good health care. Through the efforts of our social workers, and in partnership with other community agencies, we provide: --Food assistance through our food pantries in each of our locations --Nutritional counseling --Psychological counseling --Social service case management --Extended support services to people living with HIV, home visits, patient education, and community prevention programs --Trans* Health Access Program

East Bay Community Action Program (EBCAP) is a private, non-profit 501 (c) (3) corporation that provides a comprehensive array of health, behavioral, and human services to the residents of Rhode Island’s east bay. EBCAP merged with East Bay Center (EBC), a state recognized Community Mental Health Center (CMHC) and Integrated Health Home. Given that the two are recognized at the local, state, and national levels as premier providers, the transformation and merger into a single organization will greatly enhance the level of expertise and expand the breadth and depth of services that EBCAP can offer to the communities it serves. EBC has been licensed by the Department of Behavioral Health, Developmental Disabilities, and Hospitals (BHDDH) and has been nationally accredited by the Joint Commission. It is also certified by the Department of Children, Youth and Families/Department of Human Services to provide outpatient services to children and their families. With this merger, EBCAP received provisional approval by the state as an Accountable Care Entity, an in-house partnership of medical, dental, behavioral health, and social service providers working together to create a shared culture of collaboration and transformation of the health care system. The ACE will focus on supporting high quality care, enhancing health information technology, and experiencing shared savings through agreements with major Medicaid providers, such as Neighborhood Health Plan of RI. EBCAP is proud to provide a full array of services including integrated family health, behavioral, and dental services; Head Start and Early Head Start; Women, Infants and Children; family development; Healthy Families America home visiting; the Parents as Teachers Programs; Heating and Energy Assistance; Victims of Crime Act information and referral services; and much more! Citizens for Citizens, Inc. (CFC) is a Community Action Agency headquartered in Fall River, MA. Incorporated in 1965 under the Economic Opportunity Act to serve as the Community Action Agency of Greater Fall River, CFC now serves the vast majority of Bristol County, providing a wide range of services to over 34,000 clients. Major programs include LIHEAP, Weatherization, Head Start, Family Planning, and Food Pantry. CFC is funded by a variety of sources, including the Community Services Block Grant (CSBG). Through innovative manufacturing, Edesia aims to reduce the alarmingly high rates of childhood malnutrition in developing countries worldwide. Founded as a non-profit in 2009, Edesia is the one of the few U.S. producers of therapeutic and supplementary ready-to-use foods (RUTFs) for large humanitarian organizations such as UNICEF, the World Food Programme (WFP), and the US Agency for International Development (USAID). To date, we've reached over 17 million children across 62 countries. Edesia is working to eradicate malnutrition worldwide. Our vision is a world where all children have access to food and nutrition as a basic human right. We are committed to assuring that vulnerable individuals no longer suffer and die needlessly from malnutrition – a completely preventable condition. We treat all children as our own, and produce for them the lifesaving tools they need to overcome malnutrition and thrive. We act now, because the cost of inaction is too great.

Little People's College was founded in 1980 by two sisters, Sandy Rego and Joyce deSousa, who were public school teachers and mothers of preschool age children. Together they developed a unique curriculum which included the skills which children would need when they entered public school. The learning begins when a child steps through the door and continues throughout the day. Little People's College locations have been nationally accredited since 1992 and continue to pursue accreditation for the newest centers as they reach their first anniversary. There are currently 14 locations and the curriculum continues to be reviewed and updated annually to include the most current changes to local, state and national requirements. Our success has been a reflection of a dedicated, caring and hard-working staff who are committed to our mission ....to provide the best possible program of early education and care available--a program where children are loved and where children and families feel safe, secure and respected, enabling children to grow to their fullest potential: physically, emotionally, socially, cognitively and in language development. PACE-RI, pace-ri.org, is the non-profit health plan for adults aged 55 and older who want to remain at home while receiving care for complex or chronic health issues. Founded in 2005, PACE-RI provides both insurance and medical care, as well as a range of health and social services that include adult day care, transportation, and meals at sites in East Providence, Woonsocket, Westerly and Newport. Taking a comprehensive approach to well-being, PACE-RI keeps medically complex participants out of nursing homes an average of four additional years as compared to their counterparts living independently without the supports provided by PACE.
