Home Care Services of Visiting Nurse Home & Hospice is intermittent care – care provided several times a week or less for a short period of time – that requires special skills not ordinarily carried out by a lay person. Care may be provided more often if the medical condition of the patient requires it, but it is not private duty nursing. The Visiting Nurse Home and Hospice Home Care staff includes: Registered Nurses, Certified Nursing Assistants, Home Health Aides, IV Therapists, Medical Social Workers, Dieticians and Nutrition Counselors, Occupational Therapists, Physical Therapists, Speech Therapists The Palliative Care program provides home health services to patients with serious and progressive health conditions who want to continue active treatment, seeking a cure or remission. Patients in this program often have life-threatening condition and at VNHH we manage complex pain and symptoms, manage medications, coordinate care with other health care professionals, provide support for treatment decisions, ease stresses and anxiety for patients and families. Hospice is a form of philosophy; it is not a place. Hospice care is provided to patients who have advanced diseases that are considered incurable. Hospice care is about living each day to its fullest and helps to make a patient’s last months and days as comfortable and fulfilling as possible…making every moment count. Care is provided in the familiar surroundings of home or home-like setting. Care can also be provided in a long-term care setting or a hospital. The Hospice team focuses on patients’ and their loved ones’ physical, emotional and spiritual comfort. Hospice Services Include: pain and symptom management, emotional and spiritual support for patient and loved ones, personal care such as bathing, cleaning, grooming, and helping patients to sit up and move around, respite for family caregivers, medications, equipment and supplies related to hospice diagnosis (covered 100%)

HealthFirst Family Care Center, Inc. in Fall River, MA began as a Model Cities Program in 1971 in response to a need for accessible medical and dental care expressed by residents. The City of Fall River provided strong support for this program and its name was subsequently changed to the Community Development Service Center. The Center has served three generations of families. HealthFirst’s service area has grown over the past 50 years to include the Portuguese, Hispanic, Cambodian, African American and Brazilian Portuguese populations. Outer Cape Health Services (OCHS) is a is a 501(c)(3) non-profit organization with three community health center locations on Cape Cod, in Provincetown, Wellfleet and Harwich, and pharmacies in Provincetown and Wellfleet. Our mission is to provide a full range of healthcare and supportive social services that promote the health and well-being of all who live in or visit the eight outermost towns of Cape Cod: Harwich, Brewster, Chatham, Orleans, Eastham, Wellfleet, Truro and Provincetown. We serve nearly 17,000 patients per year, providing a wide range of services including primary care, specialty care, behavioral health, pharmacy, WIC and insurance enrollment assistance, and no one is denied access to services due to an inability to pay. Manage a case load of 70+ consumers and assess them for home care needs in order to keep them living independently within the community. Coordinate and manage home care services, schedules and care plans on a daily basis. Assess consumers every 3 months for continuation of care. Manage client hospitalization and skilled nursing facility discharges. Assist consumers with applying for public benefits and navigating their health insurance.

Elder Services of Cape Cod and the Islands, Inc. (ESCCI) is a private, not-for-profit community-based organization dedicated to serving older adults in the twenty-two towns of Barnstable, Dukes, and Nantucket counties. We are the federally designated Area Agency on Aging (AAA) and the state designated Aging Services Access Point (ASAP) as well as the Aging and Disability Resource Consortium (ADRC) for our three county area.
