Established in 1983, The Coordinating Center is an organization with a rich history of development and implementation of community-based, culturally competent coordinated models of care for children, adults and older adults with the most complex health care needs, disabilities, and psycho-social needs. For over 30 years, The Coordinating Center has translated national movements into ground-level, community-based programs including Transitions of Care, Medicaid rebalancing initiatives, Aging in Place, hospital readmissions reductions, and addressing homelessness. Established in 1982, Delaware Hospice is the largest and only licensed, nonprofit, community-based hospice serving Delaware and southern counties of PA. Delaware Hospice is licensed by the state of Delaware and is Medicare and Medicaid certified, and holds accreditation with the Joint Commission, ensuring that Delaware Hospice’s care meets the Joint Commission's rigorous standards of quality. Delaware Hospice is committed to being the leader in providing high quality hospice and palliative care to individuals with a limited life expectancy, their families and caregivers, and to serve as a community resource for end of life care. Delaware Hospice’s professional, multi-disciplinary team meets the patient & family's physical, emotional, practical and spiritual needs, with an emphasis on symptom management. Located on Maryland's Eastern Shore, Choptank Community Health System, Inc. (CCHS) is a private, non-profit community health center focused on providing primary medical and dental health services in Caroline, Dorchester, and Talbot Counties. Choptank's health centers provide a full range of primary medical, dental, and behavioral health services to all. CCHS is a participating provider with most major insurance companies and HMOs. In addition, a sliding fee scale is available for any patient without insurance. As a community health center, CCHS is able to use federal funding to expand current services and develop new programs to meet the health care needs in our area. We provide access to high-quality, equitable, healthcare to everyone in our community. CCHS is a nationally recognized Primary Care Medical Home, delivering higher quality and more integrated care than many health systems throughout the United States. In September 2019, Choptank Community Health was awarded a Gold-level award by the American Heart Association for improving the health and well-being of patients with high blood pressure. For decades, Total Health Care has cared for families at every life stage. Our eight health centers are located in the heart of Baltimore City. (founded in 1968) and the West Baltimore Community Health Care Corporation (1972). From 1997-2001, Total Health Care was a partner with CarePartners, a Managed Care Organization (MCO). For more than 50 years, Total Health Care has provided medical care in Baltimore.
