This was a volunteer Position for a pilot diabetes peer education program; I worked remotely from home and attended meetings onsite and numerous conference calls- landline & online.
• Duties included:
coordinating time, people, and resources (Coaching/mentoring to mainly people from diverse cultures/underserved populations); delegate tasks across the care team (Peer Eds/Mentors, family caregivers, mentees, doctors, PA.s, etc). Acted as a liaison between the clinical staff and leadership team. Tasks included but are not limited to the use of basic computer skills to develop tracking forms and report off monthly to a team of other dedicated interdisciplinary members
April 2018-program kicked off; coordinated some materials and resources for 7 mentors and 7 mentees of the program; organized teleconference calls, meetings, monthly reporting, and used project management tools for building project timeline ( e.g EXCEL spreadsheet, RACI ).
• Professional Development:- Attended two Diabetes monthly seminars (2hour meetings) –facilitated group activity at MCC with diabetic patients, Peer Mentors and families-taught by a nutritionist, Karen Bassinger at UMD Extension.
