Friend Family Health Center, Inc. (“Friend Family”) was started with the development of the Friend Family Foundation by Berman and Hannah Friend in the 1930s. Nearly 70 years later, Friend Family applied for and was designated as a federally qualified health center (FQHC) to continue to ensure access to quality care in medically underserved areas. Since then, we have opened several service sites to deliver effective care to our communities, regardless of patient income. Today, Friend Family Health Center’s delivery sites provide care services to over 30,000 patients on an annual basis. We are proud to be recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance, a designation that demonstrates our commitment to improving the quality, effectiveness, and efficiency of healthcare delivery. Although Friend Family has grown significantly, both in number of service delivery sites and patients, we remain true to our founding philosophy: to provide more medically underserved and low-income individuals and families access to high quality health services while alleviating disparities in health care outcomes.

Believing in the dignity of all people, the objective of REAL Services is to assist in establishing a community in which those we serve can maintain their independence to the maximum degree possible and find meaning and satisfaction throughout their lives. To fulfill our mission, the programs we offer enhance quality of life. Additional programs provide education for individuals and families through Options Counseling, Aging and Disability Resource Center, Lunch n’ Learns, Chronic Disease Management, Medicare Counseling, Financial Literacy, and more. REAL Services is an umbrella organization for more than 20 programs that assist elderly, disabled and low-income individuals in 12 Northern Indiana County. Our programs address the needs of education, nutrition, self-sufficiency, and protection for 30,000 people in northern Indiana each year. REAL Services has three "divisions"​: Area Agency on Aging, Community Action Agency, and Alzheimer’s Services. These three divisions serve vulnerable older adults and low-income persons of all ages. The emphasis of the Area Agency on Aging is to assist older people to maintain independence as much as possible. The emphasis of the Community Action Agency is to assist low-income families to achieve self-sufficiency. Alzheimer’s Services of Northern Indiana provides support to those suffering from dementia. REAL Services is the Area Agency on Aging for the Older Americans Act which builds a network of services for older adults and the physically disabled. Community Services programs, which support single parents and low-income families, are designed to break the cycle of dependence on public assistance. REAL Services, believing in the dignity of all people, will provide services without regard to race, age, color, religion, sex, disability, national origin, ancestry, or status as a veteran.

Suburban Access provides individual service coordination and case management for individuals with intellectual disabilities in south and west suburban Cook County, Illinois. Programs include the adult DD program, Child and Family Connections Early Intervention, and Early Intervention monitoring.

TradeWind Services, LLC (TradeWind) is a US Department of Veterans Affairs (VA) Certified Service-Disabled Veteran-Owned Small Business (SDVOSB) that has provided project-based and technical support services to an array of clients since 2005. From it's earliest days, TradeWind was set up with the infrastructure of a large business and with the notion that the company would be successful if it could be flexible and adaptable. The founding vision of the company was not to specialize in one specific niche, but rather to have multiple areas of operation. As a result, TradeWind is a licensed engineering firm and construction contractor providing both staff augmentation and project based support. TradeWind possesses and active facility clearance (FOCI) and has been providing cleared personnel for more than a decade.

Family & Children’s Center is one of northern Indiana’s major non-profit human services agencies. We serve more than one thousand children and families annually throughout St. Joseph County through our early childhood and parent education and support programs. Family & Children’s Center is dedicated to strengthening the holistic health of young families through Healthy Families, The Brain Train, counseling, and parenting programs. Our mission is to build strong families and healthy children.

FirstSteps for Kids, Inc. specializes in improving the lives of children diagnosed with Autism, PDD, Asperger’s syndrome, and related disorders. Led by Executive Director, Dr. Jennifer Harris, BCBA-D, and Clinical Director, Ms. Lisa Stoddard, BCBA, FirstSteps provides early intervention services using state of the art, empirically validated Applied Behavior Analysis treatment. FirstSteps for Kids, Inc. has been serving families in Los Angeles County, Contra Costa County, Ventura County, and surrounding areas since 2005. Innovations In Learning, PC, is a multi-faceted agency. We provide intensive ABA services to children with Autism and related disorders and Behavior Management therapy to individuals in the Waiver program, Day Services for adults with disabilities as well as outpatient services for children and adults including individual and group therapy. In all areas of our work we take pride in using empirically based interventions. We develop objective and measurable goals that allow close monitoring of treatment efficacy. Our intensive ABA program and Adult Day Services are located at our main center on Georgia Street in Merrillville.

Meals on Wheels began service in 1977 in southern Lake County, Indiana as part of the Visiting Nurse Association. At that time, meals were obtained from hospitals and nursing homes and delivered to individuals in their home. As our program continued to grow, it was necessary to open a central kitchen to prepare meals for our clients. In 1993, we purchased a building in Crown Point . We outgrew the facility in 2002 when our meal count numbers grew to over 1,200 meals daily for clients living throughout Lake County and in 2006 moved to our current location.

Boys & Girls Clubs of Elkhart County is a youth development organization with over 65 years of experience staffed by skilled, caring professionals who see potential in every child and teen that walks through our doors. Our mission is to inspire and empower all young people, especially those who need us most, to reach their full potential, as productive, caring, responsible, citizens. We are here to elevate the youth of our community and let them know that they are capable, worthy and important. We offer safe places where young people can learn, grow, and succeed. Since the doors of The Goshen Boys Club first opened in 1956, our organization has been a force for good in the lives of young people. As soon as Club members walk through our doors, they enter an environment of stability, consistency, and physical and emotional safety. Each day, we welcome more than 1,300 youth between the ages of six and 18 to our Clubhouses and school-based sites. Each day we build deeply powerful relationships with the youth we serve and open avenues of opportunity for young people across our community. Our Clubhouses run programs designed to be fun and engaging, while building skills and confidence for a successful future. Our areas of focus include Academic Success and Career Development, Good Character and Citizenship, and Healthy Lifestyles. In addition, we offer programs in Sports and Recreation and the Creative Arts and have specialized programming for Teen members. We are located in neighborhoods where the greatest need exists. We’re open when kids need us most - after school, on most school breaks, and all day during the summer. Today, we have locations in Elkhart, Goshen, Middlebury, and Nappanee and serve over 6,000 youth annually.

UNIVERSAL FAMILY CONNECTION, INC. IS A GRASSROOTS ORGANIZATION SERVING THE SOUTH AND WEST REGIONS OF CHICAGO AND ITS SURROUNDING SUBURBS, THAT WAS FOUNDED BY DR. LORRAINE BROYLS IN 1976 AND HAS BEEN OFFICIALLY INCORPORATED BY THE STATE OF ILLINOIS AS A NOT-FOR-PROFIT SINCE 1980. UFC's spectrum of programs are designed to assess the needs of the clients, provide intervention services and coordinate community services necessary to resolve issues threatening the children's lives and the family unit as a whole.
