Friend Family Health Center, Inc. (“Friend Family”) was started with the development of the Friend Family Foundation by Berman and Hannah Friend in the 1930s. Nearly 70 years later, Friend Family applied for and was designated as a federally qualified health center (FQHC) to continue to ensure access to quality care in medically underserved areas. Since then, we have opened several service sites to deliver effective care to our communities, regardless of patient income. Today, Friend Family Health Center’s delivery sites provide care services to over 30,000 patients on an annual basis. We are proud to be recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance, a designation that demonstrates our commitment to improving the quality, effectiveness, and efficiency of healthcare delivery. Although Friend Family has grown significantly, both in number of service delivery sites and patients, we remain true to our founding philosophy: to provide more medically underserved and low-income individuals and families access to high quality health services while alleviating disparities in health care outcomes.

Suburban Access provides individual service coordination and case management for individuals with intellectual disabilities in south and west suburban Cook County, Illinois. Programs include the adult DD program, Child and Family Connections Early Intervention, and Early Intervention monitoring.

Innovations In Learning, PC, is a multi-faceted agency. We provide intensive ABA services to children with Autism and related disorders and Behavior Management therapy to individuals in the Waiver program, Day Services for adults with disabilities as well as outpatient services for children and adults including individual and group therapy. Pathlights, formerly known as PLOWS Council on Aging, is a nonprofit that helps navigate and celebrate aging in 20 communities throughout the Southwest Suburbs of Cook County. We help guide adults 60 years of age and over, adults with disabilities, their caregivers, families, and friends along their best path to aging – providing resources, advocacy, programs and services needed to live with independence and dignity in the community. We focus on helping individuals who are navigating the aging process have equal access to food, shelter, care, and safety. Services offered include home-delivered meals and safety checks, in-home services, support groups, education, and Respite for caregivers, protection from abuse, access to benefits for health insurance, Supplemental Nutrition Assistance program (SNAP), energy, transportation, and property tax savings, assistance needed for safe transition from hospitals or skilled nursing facilities, shared housing, and general information about resources.

Meals on Wheels began service in 1977 in southern Lake County, Indiana as part of the Visiting Nurse Association. At that time, meals were obtained from hospitals and nursing homes and delivered to individuals in their home. As our program continued to grow, it was necessary to open a central kitchen to prepare meals for our clients. In 1993, we purchased a building in Crown Point . We outgrew the facility in 2002 when our meal count numbers grew to over 1,200 meals daily for clients living throughout Lake County and in 2006 moved to our current location.

UNIVERSAL FAMILY CONNECTION, INC. IS A GRASSROOTS ORGANIZATION SERVING THE SOUTH AND WEST REGIONS OF CHICAGO AND ITS SURROUNDING SUBURBS, THAT WAS FOUNDED BY DR. UFC's spectrum of programs are designed to assess the needs of the clients, provide intervention services and coordinate community services necessary to resolve issues threatening the children's lives and the family unit as a whole.

Commonly known for identifying and implementing best practices, CWI has administered more than 40 public and private ventures intended to improve the quality of the workforce, enhance the business climate, and ensure the realization of a higher quality of life. The Strategic Solutions Department of CWI provides consulting and project management services for customers nationally and in Northwest Indiana. The Strategic Solutions department work focuses on connecting private sector leaders with policy makers from government, education, economic development, labor, and community-based organizations to develop strategic action plans that are grounded in both quantitative and qualitative data.
