Friend Family Health Center, Inc. (“Friend Family”) was started with the development of the Friend Family Foundation by Berman and Hannah Friend in the 1930s. Nearly 70 years later, Friend Family applied for and was designated as a federally qualified health center (FQHC) to continue to ensure access to quality care in medically underserved areas. Since then, we have opened several service sites to deliver effective care to our communities, regardless of patient income. Today, Friend Family Health Center’s delivery sites provide care services to over 30,000 patients on an annual basis. We are proud to be recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance, a designation that demonstrates our commitment to improving the quality, effectiveness, and efficiency of healthcare delivery. AgeOptions is a nonprofit organization connecting older adults and those who care for them with resources and service options so they can live their lives to the fullest. Since 1974, our mission has been to sustain and promote independence for older persons in their own homes and communities. As the Area Agency on Aging of suburban Cook County, Illinois, we advocate, plan, coordinate and fund services for older adults. Together with a network of community-based senior service organizations, we connect residents with vital services such as information and assistance, community dining programs and home-delivered meals, housekeeping help, employment services, access to benefits and support for family caregivers.

DayOne PACT, formerly PACT, Inc. was founded as a 501(c)(3) non-profit organization in 1981 by families who had relatives with disabilities. Concerned about the future care of their loved ones, they established the agency to assure that there would be guardianship and related services available when they, as parents, were no longer able to function in that capacity. This service was initially provided in Chicago, but later services expanded to include the greater Chicagoland area. PACT, Inc. and DayOne Network merged on January 1, 2016. DayOne Network, founded in 1989 was a "sister" agency to PACT, providing almost the exact services, just in a different location. Providing service coordination to children and adults with developmental disabilities, and their families, DayOne PACT’s Child and Family Connections programs provide services through DuPage, Kane and Kendall Counties. Life Management Services provides trust management, trustee and guardianship services nationwide.

Our group homes provide a family atmosphere where four to eight individuals live together with 24-hour staff support. Each person has their own room, which they are encouraged to personalize. Open Door also utilizes a special dietician-directed meal program called My25. This program considers each person’s health requirements and encourages individuals to help with meal preparation. We also offer assistance for individuals seeking intermittent support. These services are for people who live in their own home or apartment but also need help every now and then. Our staff develops personal relationships with the individuals we serve and offers services in all aspects of everyday living. Open Door is focused on opening doors and developing opportunities for all of our clients. This means that we provide support in every aspect of life, assisting with personal care, doctor appointments, and medication management. Our clients also receive guidance on money management, personal shopping, and laundry assistance. The levels of support are uniquely designed around the specific needs of each person. Fun and recreation are also a big part of services at Open Door. We provide transportation to a wide variety of events and activities. Clients have the opportunity to attend sporting events, see concerts, visit museums, and take outings to local parks. We also have groups that attend dance classes, yoga, and open swim sessions. If an individual has an interest, we try our best to satisfy their need in order to promote independence, growth, and an exceptional quality of life for every person we serve.

In all areas of our work we take pride in using empirically based interventions. We develop objective and measurable goals that allow close monitoring of treatment efficacy. Pathlights, formerly known as PLOWS Council on Aging, is a nonprofit that helps navigate and celebrate aging in 20 communities throughout the Southwest Suburbs of Cook County. We help guide adults 60 years of age and over, adults with disabilities, their caregivers, families, and friends along their best path to aging – providing resources, advocacy, programs and services needed to live with independence and dignity in the community. We focus on helping individuals who are navigating the aging process have equal access to food, shelter, care, and safety. Services offered include home-delivered meals and safety checks, in-home services, support groups, education, and Respite for caregivers, protection from abuse, access to benefits for health insurance, Supplemental Nutrition Assistance program (SNAP), energy, transportation, and property tax savings, assistance needed for safe transition from hospitals or skilled nursing facilities, shared housing, and general information about resources.
