Food For Thought is a nonprofit organization that provides lifesaving nutrition support to low-income people living with COVID-19, HIV and other serious illnesses across Sonoma County. For more than 30 years, we have engaged in service to our community to provide healthy food to our neighbors in need. A group of friends in Guerneville founded FFT in 1988 amidst the AIDS epidemic to provide groceries to our seriously ill neighbors. Over time, we have shifted our focus from providing end-of-life care to providing comprehensive nutrition services, and we have expanded our eligibility to those affected by a wide range of serious medical conditions. Food for Thought provides weekly groceries including fresh produce, whole grains, and healthy protein; vitamins and supplements and nutrition education. We deliver Sonoma county-wide to clients who are homebound or homeless.

Our Vision We envision that through healthy relationships and networks of support, all people are safe, valued and prepared to create the future they want for themselves, their families and their community. Our impact strategies: increase access to resources UpValley, work with partners who can help to advance our goals, build capacity for leadership across generations, and support people in times of crisis, and help people acquire knowledge and skills to reduce and manage stress. Our Model The UpValley Family Centers operates as a Family Resource Center, which is endorsed by the California Department of Social Services as a best practice for serving families and strengthening communities. Taking a prevention-focused, community-responsive approach, our services are culturally sensitive and aim to help families and communities build on the strengths they already possess to create a future of opportunity for themselves. We serve children, youth, adults, and seniors who live and/or work in the rural communities of Calistoga, St. Helena, Deer Park, Angwin, Pope Valley, Lake Berryessa, Oakville, and Rutherford. UVFC is a 501(c)3 tax-exempt organization. Our Federal tax ID is 80-0023012.

We serve West Sonoma County with programs including employment, housing, seniors, youth, mental health & counseling services. We strengthen and empower individuals, families, and communities by providing access to critical social programs to at-risk and vulnerable populations.

Community Support Network (CSN) is a non-profit agency which was incorporated in 1982 as an outgrowth of Karibu House, founded by Sonoma State University students in 1972. CSN provides a wide range of psychosocial rehabilitative services to homeless adults with behavioral health challenges by working with families, community agencies, support groups, health care resources and service providers. Tapestry Family Services is a community-based nonprofit organization formed to develop and provide programs and services that support and advance the health and well-being of children with special needs and their familes. As the name implies, our programs are designed to weave an array of services around children and families. Like a tapestry, the service threads are linked and related, with the specific pattern of services designed to meet the unique needs of each individual child. A major goal for Tapestry is to enable local children with emotional challenges to remain in our community and thrive here. Our services are coordinated to form an integrated program of care and treatment for children and a high level of support for families - foster families, adoptive families and birth families.

Pura Vida Recovery Services offers outpatient treatment options and sober living for anyone suffering from alcohol and/or drug abuse.

Since 1996, LifeWorks has earned a reputation for providing mental health services that are informed, respectful and culturally responsive. Our vision is to build a stronger community where access to effective mental health services supports individuals and families throughout the lifespan. We work collaboratively with city, county and state programs, other service providers and the community to support the well being of Sonoma County.
