We provide one-stop accessible and coordinated care by including medical, pharmacy, case management, prevention, testing and counseling, research, patient advocacy, nutrition, housing assistance, mental health services and substance use disorder counseling on site. As we move toward expansion of primary care services, Southwest CARE Center will use the same wrap-around service model delivered within the context of our core values: compassion, communication, vision, teamwork, and respect to provide healthcare that every person deserves.
