Centers for Pain Control is an independent medical practice established in 2009, by Chetan Puranik, M.D.. Having trained in the fields of Anesthesiology and Pain Management, Dr. Puranik opened the first Centers For Pain Control office in LaPorte, Indiana. Crown Counseling was founded in Crown Point, IN, by Suzy Bonaventura-Krischke in 2008. Crown started off as a small counseling agency, but since opening, has grown to be one of the largest mental health agencies in Lake County. We offer an array of diagnostic and counseling services including, individual, family, and group. We have a staff of skilled therapists, all of whom have a Master's level degree from an accredited learning institution, who are available by appointment and offer a multitude of therapeutic modalities to fit client needs.
