Care Design NY is a Care Coordination Organization/Health Home (CCO/HH) in New York State. We provide person-centered comprehensive care management services for 27,000 individuals with intellectual and/or developmental disabilities (I/DD) in 30 New York State counties, so they may lead a quality life.
