Rochester Area Community Foundation works to improve the quality of life in the greater Rochester region by evaluating and addressing community issues, promoting effective philanthropy, and connecting donors to the critical needs of the community. Under the vitality goal, we focus on: --Supporting arts and culture; --Advancing environmental sustainability and justice; --Preserving historical assets; and --Promoting successful aging. In 2020-21, the Community Foundation awarded $34 million in grants and scholarships. We bring together health care providers, insurers, consumers and other partners to find common ground on our area’s most pressing health challenges. Using the region's most comprehensive health data, together we hammer out strategies for better care, smarter spending and healthier people. Through all of these activities, Common Ground Health seeks to promote population health improvement and reduce health care disparities in the Finger Lakes region. In this position, I combined my passion for community health with my commitment to grassroots, mission-driven work. One of my first priorities was the design and coordination of an inter-agency network of support for chronic disease initiatives in Orleans, Genesee, and Wyoming Counties. On the individual client level, I facilitated community workshops on chronic disease self-management, eventually completing a national training program and trained other community leaders in workshop facilitation. I also coached clients on self-advocacy strategies and connected them to appropriate community services, preparing them to advocate for themselves with medical providers and others.I am proud of the contributions I made to the Lake Plains mission including:Commissioning the Consumer Advocacy Council designed to provide recommendations to service providers to enhance care.Implementing a Robert Wood Johnson Foundation Grant in partnership with Population Health Collaborative of Buffalo. The grant was from the nation's largest public health philanthropic foundation. Partnering with local physician offices in target counties to distribute a Diabetes Resource Guide in an effort for providers to better educate and support patients.

The Climate Solutions Accelerator is an inclusive, nonpartisan 501(c)(3) nonprofit dedicated to creating a healthier, more equitable, and regenerative community by catalyzing local efforts to eliminate greenhouse gas emissions and address the effects of climate change.
